Do you provide "mobile" notary services?

YES. We do offer mobile services to travel to your location. Additinal fees apply . Please review our Price List.

How much does it cost to have a document notarized?

It depends on the number of signatures which require notarization. The fee is $10/notarized signature. For example: if two people are signing a single document or one person is signing two documents the price would be $10.00 x 2 or $20.00.

What Identification is required to notarize a document?

There is reasonable reliance on any one of the following forms of identification, provided it is current or was issued within 5 years:

a) An identification card or driver’s license issued by the California Department of Motor Vehicles;
b) A United States passport;
c) An inmate identification card issued by the California Department of Corrections and Rehabilitation, if the inmate is in custody in California state prison;
d) Any form of inmate identification issued by a sheriff’s department, if the inmate is in custody in a local detention facility;
(e) A passport issued by a foreign government, provided that it has been stamped by the U.S. Immigration and Naturalization Service or the U.S. Citizenship and Immigration Services;
(f) A driver’s license issued by another state or by a Canadian or Mexican public agency authorized to issue driver’s licenses;
(g) An identification card issued by another state;
(h) Oaths of Two Credible Witnesses

UNACCEPTABLE FORMS OF IDENTIFICATION INCLUDE:

*MATRICULA CONSULAR CARD
*SOCIAL SECURITY CARDS
*TEMPORARY DRIVER'S LICENSES
*STUDENT I.D. CARDS
*DRIVER'S LICENSES FROM COUNTRIES OTHER THAN USA, MEXICO & CANADA

Can a document be notarized when the signer does NOT have proper identification?

Yes; under certain circumstances. If the signer cannot provide proper I.D., the Notary Public may rely on the oath of TWO credible identifying witnesses.

Can a document be notarized if the signer is unable to write(sign) their name?

Yes. When a signer is unable to sign his/her own name, that person may sign the document by mark; the signers mark MUST be witnessed by TWO persons, who also sign the document as witnesses.

I need a certified copy of a birth, marriage or death certificate. Can you help me?

Unfortunately NOT. By law you must obtain certified copies of vital records from the county where the event took place.

I need to get a certified copy of a non-vital document. Can you help me?

Always keep in mind, California notaries acknowledge and/or affirm signers, NOT documents. IF You need to certify a copy of an original document, like a passport, background check, bank statement, etc. A copy certification by document custodian may suffice for your purposes.

Important note: It is YOU, the holder of the document (document custodian, not the notary) that will be asked to take an oath that the document is a true, complete and correct copy of the original. *Please note: you should check with the agency receiving the document whether this will be sufficient.

Live Scan FAQs

WHAT IS A LIVE SCAN?

A Live Scan is a way to take inkless fingerprints, which are then digitalized and transmitted directly to the Department of Justice (DOJ). The DOJ then checks the fingerprints against known criminal history records. A response (criminal history or no criminal history) is then sent to the agency requesting the Live Scan. If an applicant has no criminal history a response should be sent from the DOJ to the requesting agency in approximately 3 to 7 days.

WHAT DO I NEED FOR A LIVE SCAN?

Applicants must bring in three (3) copies of the “Request for Live Scan Service” form.

Acceptable primary forms of photo identification include any of the following:

  • California Driver's License
  • Department of Motor Vehicles Identification Card
  • Out-of-state driver's license

However, in the absence of a primary form of identification, a certified fingerprint roller may accept one or more of the following as SECONDARY forms of identification, but only with TWO forms of the supplemental documents noted below:

Secondary Forms of ID

  • State government issued Certificate of Birth
  • U.S. Active Duty/Retiree/Reservist Military Identification Card (000 10-2)
  • U.S. Passport
  • Federal government Personal Identity Verification Card (PIV)
  • Department of Defense Common Access Card

U.S. Tribal or Bureau of Indian Affairs Identification Card

  • Social Security Card
  • Court Order for Name Change/Gender Change/Adoption/Divorce
  • Marriage Certificate (Government issued certificate)
  • U.S. Government issued Consular Report of Birth Abroad 3
  • Foreign Passport with appropriate immigration document(s)
  • Certificate of Citizenship (N560)
  • Certificate of Naturalization (N550)
  • INS I-551 Resident Alien Card issued since 1997
  • INS 1-688 Temporary Resident Identification Card
  • INS I-688B, I-766 Employment Authorization Card

Please note: When validating the authenticity of secondary identification documents and forms, the data and information need to be supported by at least two of the following supplemental documents:

  • Utility bill (address)
  • Jurisdictional voter registration card
  • Vehicle registration card/title
  • Paycheck stub with name/address
  • Spouse/parent affidavit • Cancelled check or bank statement • Mortgage documents When supplemental documentation does not support the validation of the original identification documents, the certified fingerprint roller will not accept the form of identification as valid and will not fingerprint the applicant.
HOW MUCH DOES A LIVE SCAN COST?

There are two fees involved in the Live Scan process: the rolling (fingerprinting) fee and the background investigation fee(s).

Santa Monica Notary & Live Scan charges a $35.00 rolling (fingerprinting) fee for all applications.

The fees charged for the background investigation depends on the type of YOUR Live Scan application and what level of service is required by the agency that has requested your Live Scan.

(The background fee is the fee that the DOJ charges for researching your fingerprints to determine if you have a criminal record or not.) Background fees range from $25.00 to $110.00. This fee is in addition to the $35.00 rolling fee.

If your Live Scan form has a billing number in the “Agency Billing Number” line (this is a six digit number that starts with a “1”), that means that the agency requesting your Live Scan will be billed for the background fees and all you will need to pay is the $35.00 rolling fee. If your form does not have a billing number on it, you will be responsible for paying both the rolling fee and the background fees.

WHAT TYPE OF PAYMENT IS ACCEPTED?

We accept payment in the form of Cash, VISA, MasterCard, Discover, Money Order, Cashiers Check, Corporate or Business Checks. 

DO I NEED TO MAKE AN APPOINTMENT?

Walk-ins are welcome during our normal business hours Monday through Friday from 10AM-6PM. As a walk-in customer you will be live scanned on a first come first serve basis. Appointments are also available every half hour during normal business hours. Customers with an appointment that arrive timely supersede walk-in customers.

I HAD A LIVE SCAN DONE BEFORE, DO I HAVE TO DO IT AGAIN?

The DOJ does not share your information with other agencies. An applicant’s Live Scan form acts as a release for a criminal history response to be sent to the specified requesting agency. This means that you must submit a new application each time a criminal history is requested of you, therefore you must go through the Live Scan process again. Previous Live Scan information CANNOT be utilized.

HOW LONG DOES IT TAKE TO GET MY LIVE SCAN RESULTS?

In most cases, the results from DOJ come back within 72 hours. Due to various reasons, results can be delayed. Once the Live Scan process has been completed at our facility, please direct all inquiries to the agency handling your employment or licensing.

If a requesting agency does not have results yet , you can use the automated telephone system to check on your submission.

You will need the following information: (1) your date of birth; and (2) the 10-digit Automated Transaction Identifier (ATI) number that appears at the bottom of the Department of Justice form requesting Live Scan fingerprint background checks. The ATI number always appears in the following sequence: 1 LETTER; 3 NUMBERS; 3 LETTERS and 3 NUMBERS.

WHERE CAN I GET A REQUEST FOR LIVE SCAN FORM?

The requesting agency (the entity that is requesting the background check) will provide you with the live scan forms necessary to complete the live scan. Please also review the question below.

IF MY FINGERPRINTS ARE NOT ACCEPTED, WHAT HAPPENS?

Fingerprints are rejected on occasion for a variety of reasons. The most common reasons are the applicant's fingerprints have characteristics that are difficult to capture in the live scan process. This would include items such as cuts, scars and calluses. If an applicant has fingerprints rejected on this basis, and they were originally scanned by Live Scan Sacramento, we will rescan your fingerprints at no additional charge.

As a courtesy to our customers and vistors, we have made the following forms available to you for downloading and printing at your convenience. Please note that it is your responsibility to obtain the necessary ORI Code, Agency Set Address, Mail Code and Agency Billing Number (if applicable) that pertains to your referring agency and confirm with your referring agency that your form is filled out correctly. We are not allowed to assist you in completing your forms. 

We can not be held responsible for errors or omissions in your submission forms or selecting the incorrect form for your individual live scan submission requirements. Double check! If you submit an incorrect form you will have to repeat the process at your own expense.

TERMS, DEPOSITS & CANCELLATION By scheduling with us, you agree to our terms.

In the event the total can not be paid in advance, a deposit is required. We accept Venmo, debit or all major credit cards. NO REFUNDS are issued for any reason. Re-schedules and credit invoices may be offered. Refer to cancellation time frames below. Cancellation fees may apply.

In the event a deposit was taken vs. the payment in full, it will be deducted from the total of your services. We accept cash, all major credit cards, Venmo onsite. Processing fees may apply to credit card payments.

Quotes are provided upon request only, as documents and details must be provided to us in order to determine pricing. Live scan request forms can be uploaded directly to us for a quote by clicking here. (ATTACH live scan request form, DOES NOT NEED TO BE COMPLETED, we do not need personal info to provide you with pricing. Blank form is fine.) We are a specialty mobile company in that we travel to you. Fees are associated with our services. Fee's consist of onsite & setup fees, rolling fees, government fees.

Rejected Fingerprints- In the unlikely event that the D.O.J or other government agencies reject fingerprints, we offer complimentary re-roll & priority scheduling. (Travel fees may apply)

Request for live scan service form- Please direct all questions regarding completing your live scan application to your requesting agency prior to your appointment. In the event your applications reject for the purpose of but not limited to different variations of your name not matching or fitting, fees may apply to process and submit another application.

We require 48 hour cancellation for individuals, 7 day Cancellation for groups 2-4 and 14 day Cancellation for groups of 5 or more. Rescheduling or credit invoices may be offered. Failure to comply will result in Cancellation fees which will vary based on the quantity and services scheduled. A new deposit will be required for an additional booking.
Parking- We always attempt to located free parking when it permits. We take in to consideration the city laws, time limits and distance to walk as we carry our equipment to your appointment. Any parking fees will apply to your total.

If you neglect to have any of the documents or identification required, we must refuse service and you will lose your deposit. Additional penalties may apply.

It is important you are ready at the time we arrive to the appointment. Waiting fees will apply if, but not limited to, you are not there, not available or not ready to get started when we arrive. No exceptions. $100.00 rate per hour prorated will be added to your total. We keep a tight schedule as it is our absolute priority that we make it to every appointment in a timely manor. We very likely have scheduled appointments after yours.

No Shows and or missed appointments for individuals or groups more have Cancellation fees that will vary starting at $150.00 for individuals and going up for groups of 2 or more. There are no refunds under any circumstances. NO REFUNDS DURING THE COVID-19 PANDEMIC UNDER ANY CIRCUMSTANCES.

A credit may be issued for a future use with no expiration in some circumstances. Third-party payments are accepted however applicants are responsible for all services rendered and/or cancellations.
